Hooking up headphones to a Radio?

Q. Ive got a conversion van with passenger headphones, how do i hook these up to my stereo radio used solely for the headphones? Headphones share the ground for left and right but radio has seperate ones. the headphones were grounded to the van's frame before but this never worked, blew up radio, how should it be done? (3rd time asking this question)
im just wondering if its healthy to hook up both grounds together and them to the headphones or if it will hurt the radio...

i can only layout the fundamentals:

headphones are the same as speakers in your car. you run a left +, right +, and a shared -. you don't use the frame as - because it introduces noise from everything else that running through the car.

note that there are two types of + on the radio, one delivers power and the other delivers the music signal you don't mix the two.

you must understand the wiring scheme of your car and correctly match it up to your stereo

Okay so my phone denies access to the Radio if I haven't got a Headphone jack in the socket, I was wondering why??

Because if I use an Auxiliary Cable from phone to PC speakers, it works?
Headphones/Earphones same thing really :l

the headphone cord becomes the antenna for the radio. The speaker cord might work, but may not be long enough or the proper termination.
